Frequently, workers lose their capacity to operate in their current positions, with the result that everyone around and under them suffers. There need to be better mechanisms to downgrade workers to lower positions when necessary.
Rules need to be put in place to stop the relentless promotion of incompetent people. At the very least, any promotion should be retractable.
Implement the bed management system software and resources, system-wide for maximum utlization of resources, bed capacity and reduced (or none) wait time in emergency department; implement a Bed Czar to manage the hospital-wide process for best usage of resources and staff.

If you don't manage at least 8 people then you should not be a manager. I report to my boss who directly supervises 4 people. Her boss directly supervises 3 people. Federal managers suffer no consequences for being bad managers. The number of grievances and EEO suits filed is a red alert that something's going wrong in the office.
